Position Summary:
The Home Health and Hospice Services RN Educator plans and delivers a standardized orientation program for all new hires in Home Care and Hospice clinical positions and provides general orientation to the unit for all employees. Ensures that new employees are systematically exposed to all elements and requirements of new position, demonstrate competency, and are able to perform independently within 90 days of orientation. Prepares specific action plan if extended orientation is necessary. Remains available as a resource/consultant during the time the new employee continues to build skills and proficiency to reach full productivity. Educator will be responsible for staying up to date on regulatory requirements and accreditation standards and will ensure all orientation materials are maintained. Responsible for annual education/competency program.

Qualifications:
EDUCATIONAL REQUIREMENTS
Bachelors Nursing

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Nurse (RN) Illinois and Basic Life Support (BLS) and Driver's License and Proof of Auto Insurance. Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

EXPERIENCE REQUIREMENTS
Three (3) Home Health and One (1) Medical/Surgical Patient Care

OTHER REQUIREMENTS
Ability to communicate, both verbally and written, with patients, clients, management, and other employees. Ability to negotiate and collaborate with peers and other health team members is essential. Proficient in data entry and use of a computer, including word processing and Excel spreadsheets. Develops personal organizational techniques to promote the highest level of productivity. Possess the ability and initiative to plan for and complete daily activities with minimal direction. Ability to problem solve, effectively coach and mentor, and possess team building skills.

Essential Functions:

  • Assist management team to improve new employee satisfaction in his/her job; gain mastery, confidence and competency of performance in new position; decrease turnover due to new employee dissatisfaction.
  • New orientees are progressed systematically and as expediently as possible. Orientees routinely demonstrate ability to perform competently and independently within 90 days of hire. Orientees demonstrate proficiency and full productivity within 6 months
  • Orientees work is planned in advance with sufficient detail to minimize downtime and promote expedient completion.
  • Uses collaborative, respectful, problem solving communication tone, style and words.
  • Assesses and problem solves using Home Care and Hospice knowledge (Regulatory Guidance, Accreditation Guidance, Conditions of Participation) to promote desired outcomes in response to regulatory changes and customer concerns.
  • Develops and implements systematic orientation process for all clinical positions in home care and hospice orientation.
  • Develops and coordinates annual education/competency program for clinicians in home care and hospice.
  • Maintains ongoing knowledge of all job functions (Intake, Field Clinicians, Admission Nurse) performed within home care and hospice as well as maintains a thorough working knowledge of all of the systems (Regulatory Guidance, Accreditation Guidance, Conditions of Participation, EMR) utilized to perform the work in home care and hospice.
  • Demonstrates a growing and applied knowledge base in areas of Medicare Conditions of Participation, ACHC standards, standards of care, patient care resources, references, protocol, and procedures.
 
  • Assesses and problem solves using Home Care and Hospice knowledge (Regulatory Guidance, Accreditation Guidance, Conditions of Participation) to promote desired outcomes in response to regulatory changes and customer concerns.
