Position Summary:
The Hospice Supervisor supervises patient care staff to ensure the provision of patient-focused care. Ensures that care is managed through appropriate utilization, clinical interventions and care planning, regulatory compliance, and that patient care outcomes are attained. Provides clinical assessment, education, care planning and monitoring of response to therapies and therapeutic outcomes to patients in their home or alternative site. Assumes full responsibility for the department in the absence of the manager. Hours may vary depending upon census and program need.

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Nurse (RN) Illinois and Basic Life Support (BLS) and Driver's License and Proof of Auto Insurance and National Nursing Certification within three (3) years of start date in position. Employees hired into this role prior to 1/1/2020 will be grandfathered and have four (4) years from start date in position.

OTHER REQUIREMENTS
Must have knowledge or willingness to learn Home Care Processes for on-call/rotations with other managers. Ability to communicate, both verbally and written with patients, clients, management and other employees. Ability to negotiate and collaborate with peers and other health team members is essential. Proficient in data entry and use of a computer, including word processing and Excel spreadsheets. Develops personal organizational techniques to promote the highest level of productivity. Possesses the ability and initiative to plan for and complete daily activities with minimal direction. Possesses the ability to problem solve, effectively coach and mentor and have team building skills.

Essential Functions:

  • Supervises all patient care staff to ensure that provision of care is given in compliance with state and federal laws and according to standards of professional practice. This is done through supervisory visits, competency demonstration and documentation review.
  • Guides, directs and motivates interdisciplinary team members to achieve maximum outcomes of patient care through clinical care, team conferences, patient care conferences and other communication with team members.
  • Interviews and participates in hiring decisions and in progressive disciplines of all patient care staff and conducts yearly performance appraisals with input and review of clinical manager.
  • Serves as a coach and mentor to clinical staff regarding patient care issues including appropriate utilization, clinical intervention, documentation, care planning, attaining clinical outcomes and regulatory compliance.
  • Performs all essential functions as a Hospice Field RN as needed to ensure optimum productivity, efficiency and completion of all necessary tasks. Provides back-up for on-call nurses.
  • Participates in the administration on-call rotation.
  • Develops and maintains knowledge base of current and applicable federal and state laws, regulations and expected changes in industry.
  • Demonstrates a growing and applied knowledge base in areas of standards of care, patient care resources, references, protocol, pertinent drugs, significant lab values, and procedures.
  • Assists with the orientation, continuing education, and the development of competencies for new and current staff; collaborates with the Home Services Educator, staff, and other leadership.
  • Collaborates with the management team and staff in the development and review of departmental policies and procedures.
  • Participates in performance improvement activities including benchmarking, record review, data collection/analysis/reporting, facilitation and/or participation on various teams/committee, and implementation of various initiatives.
  • Assesses and problem solves using Hospice knowledge (Regulatory Guidance, Accreditation Guidance, Conditions of Participation) to promote desired outcomes in response to regulatory changes and customer concerns.
  • Monitors employee performance and ensures timely communications for the purpose of goal setting,
  • performance evaluations, wages increases; Provides coaching and one-to-one feedback to hospice staff.
  • Assumes responsibility for personnel related duties that include interviewing, disciplinary
  • actions, payroll reporting, scheduling paid leave, and enforcing requires rules and regulations for approximately 15 Direct Reports.
  • Maintains ongoing knowledge of all job functions (Intake, Field Clinicians, Team Approach with Social Work, Chaplain, and Volunteer) performed within hospice as well as maintains a thorough working knowledge of all of the systems (Regulatory Guidance, Accreditation Guidance, Conditions of Participation, EMR) utilized to perform the work in hospice.
  • Demonstrates a growing and applied knowledge base in areas of Medicare Conditions of Participation, ACHC standards, standards of care, patient care resources, references, protocol, and procedures.
